Penile enhancement: filler and fat grafting (lipofilling)

Increasing penile girth with hyaluronic acid or autologous fat: how the two techniques work, what you can expect, what the risks are and when treatment is not indicated. With what the scientific literature really says.

What it is

Medical-surgical penile enhancement (in the literature Penile Girth Enhancement) is a group of procedures that aim to increase the girth, that is the thickness, of the penile shaft. It does not act on length, which requires different techniques (see surgical penile lengthening).

According to the International Society of Aesthetic Plastic Surgery (ISAPS), penile enhancement procedures recorded worldwide rose from 10,053 in 2014 to 11,703 in 2015, an increase of more than 16% in a single year. The scientific literature studying their effectiveness and safety has grown in step: the results are encouraging, but most studies are still of limited quality. On this page we say so openly.

Hyaluronic acid filler

How it works

I use a cross-linked hyaluronic acid with high density and viscoelasticity, formulated to hold its shape in the tissues. It is a Class III CE-marked medical device whose instructions for use expressly include temporary penile volume enhancement: I therefore use it in line with the manufacturer's indications, which also recommend blunt cannulas. The product is spread evenly along the shaft with blunt cannulas, with a rounded tip, in the subcutaneous plane, superficial to Buck's fascia: it therefore stays outside the corpora cavernosa, the urethra and the dorsal neurovascular bundle. Unlike a needle, a blunt cannula slides between the tissues, pushing vessels and nerves aside instead of piercing them: this reduces the risk of bruising and of accidental injection into a vessel, and only a few entry points are needed.

What the studies say

It is the most studied injection technique. The first prospective study (Kwak, 2011) documented a significant increase in girth maintained at 18 months, with no inflammatory reactions. A randomised multicentre study of 74 patients (Yang, 2020) reported an average increase at 24 weeks of 2.1 cm with hyaluronic acid and 1.6 cm with polylactic acid, with improved satisfaction in both groups. A 2026 prospective study (Park, 2026), carried out with the same type of product I use, followed 20 men for one year: with an initial injection below 20 ml and small touch-ups at 1–2 weeks, the girth increase remained stable at 48 weeks, satisfaction with appearance improved in 80% of cases and there were no serious adverse effects. The European guidelines (EAU) report patient satisfaction between 78 and 100% and no serious side effects in published studies.

What the studies say

The European guidelines report a mean girth increase of between 2 and 3.5 cm, with more than 75% of patients satisfied. They specify, however, that there are still not enough long-term data. The main limitation is biological: part of the grafted fat does not take and is resorbed in the first months, to a degree that varies from person to person and cannot be predicted precisely. The fat that survives stays in place, but the final result is only assessed after a few months and may require a second graft.

Filler or fat grafting (lipofilling)? There is no single best technique. Filler has more studies behind it, is reversible and does not require surgery, but has to be repeated. Fat grafting uses your own tissue and may last longer, but the result is less predictable and cannot be undone. The choice depends on anatomy, expectations and the availability of fat to harvest, and is made together during the consultation.

Risks and side effects

In the published studies serious complications are rare and undesirable effects are mostly mild and transient. There is no such thing as zero risk, and the risks need to be known before deciding:

An important warning. The European guidelines strongly recommend not using silicone, paraffin or petroleum jelly to increase penile thickness. These substances, often injected “do-it-yourself” or outside healthcare facilities, cause granulomas, severe infections, ulcers, permanent deformities and tissue necrosis, which may require complex reconstructive surgery. Every treatment must be carried out by a specialist doctor, with products certified for medical use.

The psychological side: when treatment is not indicated

The perception of one's own size is often influenced by media and pornographic models that distort the idea of “normal”. As a reference, the European guidelines report an average girth of about 9.3 cm at rest and 11.7 cm when erect.

During the consultation it is essential to distinguish a legitimate aesthetic wish from two specific conditions:

The European guidelines strongly recommend not performing filler or fat grafting (lipofilling) in men with penile dysmorphic disorder: in these cases the treatment does not resolve the distress and the first recommendation is a course of psychological support. This is also why, at times, the most honest outcome of a consultation is to advise against the procedure.

Frequently asked questions

By how many centimetres does the girth increase?

In studies the mean increase is about 2–4 cm with filler and 2–3.5 cm with fat grafting (lipofilling). The individual result depends on the amount of product or fat used and on the elasticity of the starting tissues, and is discussed realistically during the consultation.

Do filler or fat grafting (lipofilling) also increase length?

No. Both techniques increase girth. At rest the penis may look fuller, but erect length does not change. For length, different surgical techniques exist.

How long does the effect of penile filler last?

Hyaluronic acid is gradually resorbed. In studies the effect is documented for up to 18 months, with wide individual variability; top-up sessions can be planned to maintain it.

Is fat grafting (lipofilling) permanent?

In part. A share of the grafted fat is resorbed in the first months; the part that takes stays in place. The final result is assessed after a few months and a second graft is sometimes needed. The guidelines note that long-term data are still scarce.

What happens if I am not satisfied with the filler result?

Hyaluronic acid can be dissolved with the enzyme hyaluronidase, which makes it possible to correct an asymmetry or remove the product. Fat from fat grafting (lipofilling), on the other hand, cannot be dissolved.

Does the treatment affect erection or fertility?

The product or fat is placed in the superficial plane, above Buck's fascia, without involving the corpora cavernosa, the urethra or the vascular and nervous structures responsible for erection. No effects on erection or fertility are reported in the studies.

Does it hurt?

Filler is performed under local anaesthesia or with anaesthetic cream and discomfort is generally modest. Fat grafting (lipofilling) is performed under local anaesthesia, possibly with sedation. In the following days swelling and mild soreness are common.

What are the main risks?

Transient swelling and bruising, temporary reduction in sensitivity, asymmetries or nodules and, rarely, infection. Serious risks are linked mainly to unauthorised substances such as silicone or paraffin, which must never be used.

    Why it matters: it is one of the few randomised studies available and directly compares two fillers; it also shows that follow-up remains short.

    Why it matters: it is the most recent study and was carried out with the same type of product I use; however, it is small and has no control group.
