Glans corona filler: a complete guide

What it is, who it is suitable for, how the result is measured (IELT) and what the scientific literature really says about its effectiveness and safety.

What it is

Glans corona filler (in the literature Glans Penis Augmentation, GPA) is a non-surgical male intimate medicine procedure performed as an outpatient. It involves targeted injection of hyaluronic acid along the coronal edge of the glans, with a twofold aim: to reduce tactile hypersensitivity of the mucosa and to slightly increase the circumference of the glans.

How it works

The treatment acts in two ways:

I use cross-linked hyaluronic acids with high viscoelasticity, formulated to hold their shape in the tissues and to be gradually reabsorbed. For this reason the effect lasts over time but is not permanent.

  • Temporary effect: a top-up may be needed
  • Not suitable if the cause is psychological, hormonal or urological

Who it is suitable for (and who it is not)

Effectiveness depends above all on correct patient selection. The filler may be suitable for:

An effect often reported by couples concerns their sexual rapport: the fuller corona increases stimulation during penetration and may help the partners to be better in sync.

When it is not the first choice. Premature ejaculation does not always have the same origin. If psychological or relationship factors predominate, or there are hormonal or urological causes (for example thyroid problems or prostatitis), the filler does not address the problem at its root. In these cases the patient is guided towards a different or combined approach.

IELT: the parameter we use to measure the result

What is IELT and why do we measure it?

IELT (Intravaginal Ejaculatory Latency Time) is the time that elapses between the start of penetration and ejaculation. It is the parameter used by the scientific community, and by the International Society for Sexual Medicine (ISSM) in its official definition, to assess the severity of the condition and the effectiveness of treatments.

In published studies, glans corona filler increased the mean IELT by roughly 2–4 times compared with the baseline value, with wide differences between one patient and another.

Time alone is not enough for a diagnosis. According to the ISSM definition, premature ejaculation is diagnosed only when a short latency is accompanied by poor control over the reflex and by personal or relationship distress. There is no “right time” that is the same for everyone: the goal is defined together during the consultation.

How it is measured before and after the filler

To measure it at home all you need is a stopwatch, started at the beginning of penetration and stopped at ejaculation. It is best to repeat the measurement over at least 3–5 occasions of intercourse and calculate the average.

What the scientific literature says

The technique was proposed in Korea in 2004. In the first study (139 patients, 65 of whom were treated with the filler alone) the IELT rose from about a minute and a half to about 4.7 minutes at six months, with 75% of patients and 62% of partners satisfied. Later studies reported similar results:

What we do not yet know. The most recent systematic review (2024, 10 studies) confirms effectiveness but rates the quality of the evidence as low: small samples and non-uniform assessment tools. The European Association of Urology (EAU) guidelines recognise a modest but significant increase in IELT and recommend using the filler “with caution” compared with more established treatments, because further safety studies are needed. It is right that you know this before choosing.

Risks and side effects

Recovery

Recovery is generally quick: normal daily activities are resumed immediately, while for sport you wait 24 hours. It is advisable to abstain from sexual intercourse for 7–14 days; the exact period is defined by the doctor according to how healing progresses.

Frequently asked questions

Does glans corona filler hurt?

It is performed under local anaesthesia (anaesthetic cream or a nerve block), so discomfort during the injections is generally minimal. In the following days slight swelling or local soreness may appear, which resolve on their own.

How long does the effect last?

Hyaluronic acid is gradually reabsorbed, so the effect is not permanent and its duration varies from person to person. In the study with the longest follow-up (38 patients followed for 5 years) the effect had partly diminished over time, but values were still higher than at the start. Periodic top-ups are also described in the literature.

Is the treatment reversible?

Yes. The filler is reabsorbed on its own over time and, if necessary, it can be dissolved with a specific enzyme, hyaluronidase.

How soon can I resume sexual intercourse?

Generally after 7–14 days. The precise period is indicated by the doctor according to the amount of product injected and how the first few days go.

What are the main risks?

The studies mainly describe transient effects: swelling, bruising, mild pain or discolouration, which generally resolve within 2 weeks to 1 month. In the largest series (over 4,300 treatments in Korea) pain or sensory changes were reported in 0.5% of cases. However, the European guidelines (EAU) point out that serious complications can also occur and that further safety studies are needed.

Does it work for all forms of premature ejaculation?

No. The studies mainly concern premature ejaculation linked to glans hypersensitivity. If the problem has a mainly psychological or relationship component, or a hormonal or urological cause, the filler is not the first choice. The consultation exists precisely to understand which situation you are in.

How does the filler’s “cushion” mechanism work?

The hyaluronic acid injected into the corona creates a thickness of a few millimetres between the surface and the underlying nerve endings. In this way it reduces the most intense tactile stimuli during intercourse.

Does the filler abolish sensitivity of the glans?

No. The aim is to moderate hypersensitivity, not to anaesthetise the glans: sensitivity and pleasure remain. Changes in sensitivity are possible but rare, and are among the risks discussed during the consultation.

Can it be combined with other treatments?

Yes, when there is an indication. It can be considered together with girth enhancement of the shaft with hyaluronic acid or fat grafting (lipofilling), or with frenuloplasty if a short frenulum is also present. The choice is made during the consultation.

How can I measure my IELT at home?

In studies a stopwatch is used, started by one of the partners at the beginning of penetration and stopped at ejaculation. It is best to measure it over several occasions of intercourse, at least 3–5, and take the average. An estimate made by the patient is also considered a valid reference under the ISSM definition.

Does IELT also apply to anal or oral intercourse?

By definition IELT refers to vaginal penetration, and this is what the studies have been conducted on. The mechanism of the filler, namely the reduction of glans hypersensitivity, applies to any stimulation, but there are no specific data for other types of intercourse.

    What it covers: follows 38 patients for 5 years. Volume and IELT decrease compared with the 6-month check, but remain above baseline values; at 5 years 76% of patients and 63% of partners are still satisfied, with no serious adverse reactions.

    Why it matters: it is the only long-term duration data, useful for understanding that the effect diminishes but does not disappear suddenly.

    Why it matters: it confirms the Korean results in a different population, but in few patients and with short follow-up.

    What it covers: a randomised study of 30 patients comparing the filler with an injection of saline solution. At one month the IELT increases by a median of 2.6 times with hyaluronic acid and 1.1 times with saline solution.

    Why it matters: it is the comparison with a “placebo” control: it shows that the effect is not due solely to the patient’s expectation.

    What it covers: a questionnaire sent to 56 Korean urologists, 36 of whom perform the treatment, for a total of 4,344 procedures. Pain or sensory changes in 0.5% of cases, recurrence of premature ejaculation in 4.7%.

    Why it matters: it is the largest real-world case series available and gives an idea of how often complications occur in clinical practice.

    What it covers: a review of the available studies: IELT increases by 2.4 to 4.5 times, with mild and transient side effects. The authors, however, emphasise small samples and non-validated assessment tools, and consider the treatment still experimental.

    Why it matters: it summarises both the results and the limits of the evidence in a balanced way, and is the source of the “2 to 4 times” figure.

    What it covers: a systematic review of 10 studies: IELT increases of 1.5 to 8.5 times, side effects between 0 and 30%, mild and resolved within a month. Only one study in ten has a low risk of bias.

    Why it matters: it is the most recent synthesis: it confirms effectiveness but openly states that the quality of the evidence is still low.
